There's a much faster route to a Flawless Horadric Gem than endlessly running ordinary dungeons, though it depends on a weird Escalation reset. If you'd rather avoid chasing the setup, Diablo 4 Items can also cover the gap. For players who enjoy finding broken-looking farm routes, this one is worth trying before it gets patched.
Start with the right War Plan choice
You need Branching Pathways on the War Plan. No workaround, really. It gives you three portal choices while moving through Nightmare Escalations, which is what lets you fish for a profitable third floor. Get an Escalation Sigil by transmuting five normal Nightmare Sigils in the Horadric Cube, or hope for one from a Strongroom boss at higher Attunement.
Build the third-floor setup
Clear floors one and two, then choose portals that lead toward Gem Reserve and Dark Omen.
Why those affixes matter
Pick a specific reserve, such as Ruby Reserve or Amethyst Reserve, not the generic gem option. The named reserve pays far better. Dark Omen is the other half of the trick. When its stalking boss turns up, kill it. On high Torment levels, that kill can throw out roughly 750,000 fragments. Chests help, but the ambush is the real payday.
Reset instead of finishing
Once the third floor has both affixes, open every Horadric chest and deal with the Dark Omen boss. Don't touch the final boss. Don't take the exit. Teleport to town, enter a nearby normal dungeon, then open the Sanctuary map and teleport back to the active Escalation Dungeon. The third floor should reload with fresh chests and another boss spawn.
What the loop actually saves
It sounds fiddly at first, but after a couple of resets it becomes muscle memory. You're not farming the normal dungeon; you only enter it to remove the return portal and force the Escalation instance to behave differently.
| Farm method | Best reward source | Time pressure | | Escalation third-floor loop | Dark Omen boss and Gem Reserve chests | High until patched | | Regular dungeon runs | Small chest and monster drops | Low but very slow |
Keep the run from falling apart
Leave immediately after looting, because clearing the normal dungeon wastes the valuable reset window.
Materials still matter
Twenty-five million fragments only gets you to the crafting stage: 25 Grand Gems become 25 Horadric Gems, then one Flawless Horadric Gem. You'll still burn gold and Forgotten Souls, so don't spend every session on fragments alone. If the reset gets hotfixed or you simply don't fancy the loop, grabbing D4 items cheap is a practical fallback for keeping your build moving.
